The third annual Blake Bortles Foundation Charity Golf Tournament will be held on
Sunday and Monday, June 24 and 25, 2018 in St. Augustine, Florida. The event will kick
off with a concert series on Sunday, June 24 at the World Golf Hall of Fame, followed by
the golf tournament which will return to the beautiful King & Bear Golf Course at the
World Golf Village on Monday, June 25.

The tournament was created in 2016 as a main fundraising platform for the Blake Bortles
Foundation. Our foundation supports two areas of need in the Jacksonville and
Oviedo communities:

   Children and adults with intellectual and developmental disabilities (IDD),
		 to which we provide opportunities through partnerships with local organizations
		 to help these amazing individuals pursue full, independent lives.

   First responders, as they continue to strengthen and protect our citizens,
		 while carrying out their vital, lifesaving work.

The Blake Bortles Foundation Charity Golf Tournament is a two-day event that combines
music, golf, local flavors and a first-class good time. Our Sunday evening event will feature a
live performance from nationally acclaimed artists, local flavors from North Florida’s favorite
spots, and a live and silent auction. All centered around gearing you up for Monday’s golf
challenge. The golf tournament is not your average time on the green as you’ll be competing
with Blake, his Jaguars teammates and other celebrity friends along the course. All played
with end goal of raising awareness for our specific areas of need.

The Checklist
1. REGISTRATION
Please fill out the REGISTRATION FORM on the last page of this brochure and mail or email it
along with payment to the address listed on the form. This will secure your team for the 2018
Blake Bortles Charity Golf Tournament. Once the tournament office receives your registration
form, you will receive a confirmation email.

2. HOTEL ACCOMMODATIONS
Rooms are available at the Renaissance Resort at World Golf Village, which is where the
live music and Pairings Party will take place.

3. payment
Payment should be sent in with your registration form. If other arrangements need to be made,
please contact the tournament office.

4. register golfers
The tournament director will contact you regarding golfer names no later than June 15, 2018.

5. tournament information packet
The tournament information packet will be sent out to the team contact on June15, 2018.
This will remind you of when and where all of the festivities will take place.

Live and silent auctions taking place throughout the night with dinner and cocktails

Maggie Rose and special guests.

The course
World Golf Village’s King & Bear course features design collaborations by four World Golf Hall of Fame
members and consistently receives a four-and-a-half rating (out of five) by Golf Digest. The course has
hosted Professional tournaments including the Liberty Mutual Legends of Golf, a Champions Tour
event, and Shell’s Wonderful World of Golf, as well as other nationally-televised events.
