21IMPORTANT INFORMATION
Missed Activities: In the event that a participant is unable to
attend an activity, we regret that a make-up activity, refunds, or
credits cannot be offered.
Payment: Full payment of activity fees must be made at the
time of registration. Payment by cash, cheque or money order
are acceptable methods of payment for all activities.
Health & Safety: We reserve the right to ensure the health and
well-being of all participants. Participants will be sent home, if
there is infectious illness identified that may pose a risk to either
staff or participants.
Waiver of Liability: We shall not be liable, either directly or
indirectly, for any claims, damages, costs, expenses, including
but not limited to personal injury, property damage or lost or
stolen property, arising from or connected with participation in
any activity contemplated by a registration, misrepresentation,
breach of statutory duty, breach of the Occupier’s Liability Act
and /or breach of contract on the part of the organization.
Inclement Weather: In the event of inclement weather we
may need to cancel for your safety and will make every effort
to add an additional class at a later date to make up for the
missed class.
Cancellation: We understand that there are times when
cancellation is necessary due to personal reasons, health and
inclement weather. Please be aware that there are times where
full refunds can not be provided due to pre-purchase of tickets.

Covid Protocols
Community Living Haldimand works closely with the Haldimand Norfolk
Health Unit as well as Public Health Ontario to ensure current Infection
Protection and Control guidelines are in place. We also follow the Ontar-
io Government ‘Roadmap to Reopening’ plan to ensure the safety for
our Passport recipients and our staff. As provincial/regional restrictions
change we will continue to update this information accordingly.
Safety Measures include:
Social/physical distancing of 2 meters where possible.
Hand hygiene.
Agree to wear a mask when in enclosed space such as the vehicle. If
you do not have a mask, one will be provided.
Enhanced interior cleaning of the vehicle before use.
You will be asked to provide confirmation of being double vaccinated
against COVID-19 before taking part in activities.
You will be called prior to your outing by one of our Passport team
who will confirm that you pass the Provincial COVID-19 screening
questions which include:
Do you have any of the following symptoms: fever 37.8C or
higher; barky cough; shortness of breath; runny nose; unusual
headache; digestive problems unrelated to existing problems,
muscle/joint achiness; extreme tiredness or falling?
Is anyone living with you experiencing COVID symptoms or has
tested positive in the last 14 days?
Have you travelled outside Canada in the last 14 days and been
told to quarantine?
Has a Dr. or Health Unit told you to stay home to self-isolate in
the past 14 days?
If you pass these questions you may go on your outing.
If you do not pass the questions, please understand that you cannot go
on the outing as it is a risk to others. With the last minute cancellation,
no fees will be reimbursed.
23CANCELLATION & WITHDRAWAL
GUIDELINES FOR ACTIVITIES
AND EVENTS
Event cancellation by agency: Cancellation of an event or activity may
occur due to low enrollment or unforeseen circumstances such as
inclement weather. You will be provided with an alternate activity, date
or a refund*.
Event/activity cancellation by participant:(with 14 days notice or
more) Should you need to withdraw from an activity after registering and
submitting payment you must do so at least 14 days before the planned
date of the activity or the beginning of the session.
If the withdrawal is due to medical reasons ( covid policy on pg 23) a medi-
cal note should accompany your request for refund. Request must be
made by no later than midpoint of a 10-11 week session.
If less than 14 days notice, in both cases above, you will receive a 50%
refund less a $35 administration fee*.
Please note: all requests for a refund must be made in writing
accompanied by the original receipt. Non– attendance does not constitute
notice of withdrawal.
*If tickets have been preordered and paid for; we may not be able to refund costs.
All efforts to find replacements will be made.
1:1 Support purchase needs to be cancelled 48 hours or more to be subject to an ad-
ministration fee of $35 and pre paid purchases.
If less than 48 hours, the above applies plus 3 hours.
