Eligibility & Enrollment
Who can elect benefits and when and how to elect them

                                                           Making Changes During the Year
                                                           IRS regulations state that you cannot change your

                                                           You must access the benefits enrollment portal through
                                                           Workday to initiate the life event change. Per IRS
                                                           guidelines, changes must be made within 30 days of the
                                                           actual event date. Note: The system will not allow you to
                                                           enter your qualifying life event change before the event
                                                           occurs.

We offer a Preferred Provider Organization (PPO) medical plan, through Blue Cross Blue Shield, to ensure you have extensive
coverage for all your medical needs. You can take comfort in knowing this plan has you covered — whether it’s for routine care
or you have an unexpected medical need like surgery or hospitalization. The best part? Sage fully covers your calendar year
deductible through the Health Reimbursement Account.

Health Reimbursement Account
For your medical deductible

The Health Reimbursement Account (HRA) is 100% funded by
Sage and provides direct reimbursement to your providers for your
calendar year deductible.

Using an Out-of-Network Dentist
You can receive dental care from a dentist outside the Blue Cross Blue Shield of Massachusetts network. Keep in mind that when
you do, you will be responsible for any amounts over what is considered Reasonable & Customary (R&C). You may be responsible
for any difference between the dentist’s actual charge or the allowed charge, whichever is less. You are also responsible for your
deductible and coinsurance (if applicable), and charges beyond your calendar-year or lifetime benefit maximum.

Flexible Spending Accounts
Savings for health care and dependent care expenses

Flexible Spending Accounts (FSAs) allow you to set aside money on a pre-tax basis through payroll deductions to help pay
for certain out-of-pocket health care and/or dependent care expenses. Beginning January 1, 2021, HealthEquity will now
administer our FSAs.

Disability Benefits
Providing income replacement when you can’t work

  Reliance Standard Short-Term and Long-Term Disability

  Disability insurance helps protect you and your family if you become disabled due to illness or injury and are unable to
  work. Short-Term Disability (STD) and basic Long-Term Disability (LTD) insurance are provided by Sage at no cost to you.
  Eligible employees are enrolled automatically.

  Short-Term Disability

  STD provides 100% of your base salary for up to a maximum of 13 weeks for an approved disability.
  If eligible and approved, benefits begin on the 1st day for an injury and on the 8th day for an illness. Note: There is no
  elimination period for disabilities caused by an accident.

  Long-Term Disability

  LTD replaces 60% of your base salary, up to $12,500 per month.
  The elimination period is the later of 90 days or the end of your STD payments.

Note: Reliance Standard manages the claim review and processing for all STD and LTD claims.

A Note About LTD Premiums
While Sage pays the premium for LTD, you have two options that can impact your benefits.

— Option 1: Sage pays the LTD premium, which means your LTD benefit is taxable in the event you file an LTD claim.

— Option 2: You pay the taxes (reflected on your paystub) on the Sage-paid LTD premium, which means your LTD benefit is
  tax-free in the event you file an LTD claim.

Retirement Savings

  401(k) Savings Plan through Principal

  Principal Plan Features      Description

  Eligibility                  You are eligible to participate in the plan as of your date of hire.

  Your Contributions           Contribute up to the IRS annual limit ($19,500 for 2021) on a pre-tax and/or Roth 401(k)
                               (after-tax) basis. If you are age 50 or older, you may make additional “catch-up” contributions
                               ($6,500 for 2021) per year. Limits subject to change by IRS.

  Company Matching             Sage matches 50% up to the first 6% you contribute.
  Contributions                Note: You will receive the company match each pay period for the year. Sage has a true-
                               up feature on the match, which ensures you receive the maximum company match if you
                               contribute at least 6% of your eligible compensation for the year—regardless of the timing of
                               your deposits. With the true-up, Sage will look back at your Year-to-Date (YTD) contributions.
                               If your contributions qualify for any match that you didn’t receive on a per-pay-period basis,
                               Sage will add money to your Plan account to match your YTD contributions.

  Vesting                      You are always 100% vested in your contributions. You will vested in company contributions
                               25% per year; 100% after 4 years.

  Investments                  You can invest your contributions and account balance among a variety of investment fund
                               choices offered through the Principal.

   — Ongoing Account Access: Go to principal.com, select Log In and choose Personal (enter your username and password).

 Enrollment & Offering Periods      ESPP periods are for six months:

                                    The open enrollment period is approximately 10 days during the month (December and
                                    June) preceding the first day of the ESPP period.

 Your Contributions                 Eligible employees may elect to contribute between 1% and 10% of their base salary
                                    during the offering period. During the offering period, employees are locked into that
                                    percentage, but may withdraw from the plan at any time.

 Purchase                           Purchase shares at a discount of 15% from the fair market value of Sage stock at the
                                    close of business on the first day of the offering period and the last day of the offering
                                    period, whichever price is less.
