that is open to the public requires to refuse
entry or service to those not wearing a mask /
face covering. While in the center, we will ask
you to wear your mask at all times.
From the Director:
I recently had the opportunity to tour Oak Street
Health, located at 102 W. Pierson Rd. It is a primary
care facility accepting all Medicare recipients.
The staff were warm and welcoming and the facility
itself appeared completely updated.
Along with primary
care services, Oak
Street Health offers
the following
services:
• Choice of

• Rides to and from center appointments, (includes Depart 1:00pm / Return 3:00pm
