Registration INFORMATION
                              REGISTRATION POLICY
                              Please complete the registration and waiver forms on PAGES 16 - 18 or REGISTER ONLINE at apm.activecommunities.com/
                              abingtonparksandrec. You will be considered registered for a program only after the proper forms and fees are received by
                              the Parks and Recreation Department before the stated deadline. You will not receive a confirmation unless otherwise noted.
                              Registrations without accompanying fees will NOT be accepted. Please note that there are some programs which require
                              REGISTRATION IN PERSON. If you mail a registration in for an “in person” program, the registration will be entered at the
                              end of the work day. Non-residents may have a delayed registration start date and/or a higher registration fee.

                              PAYMENT FOR PROGRAM
                              Payment is due at the time of registration. You may pay by Visa, Master Card, Discover, American Express, branded debit card,

                              cash, or check. Checks should be made payable to “ABINGTON TOWNSHIP”. All returned checks will be assessed a fee of
                              $25.00. If it is a Briar Bush Nature Center program (pages 6 - 8), please make checks payable to: “FRIENDS OF BRIAR BUSH”.
                              Separate checks are encouraged. List all participants in the family on one form but send a separate check for each program. This
                              way if one program is full, it does not affect everyone, since payment must be processed in order to be considered registered in
                              a program.

                              CANCELLATION OF PROGRAM
                              Weather and other conditions may force us to cancel or postpone a program. The Department will make every effort to make
                              the decision as early as possible. If there is any question about the status of a program, please call our office at 215-576-5213
                              between 8:00 A.M. and 4:00 P.M.

                              REFUND POLICY
                              A 20% service fee will be charged for ALL refund requests. Requests must be made at least five (5) business days before a
                              program or thirty (30) days before a trip is scheduled to begin. There will be no service fee if the refund amount is applied to
                              a future program within the calendar year or the program is cancelled by the Township. Refunds will not be issued once the
                              program begins, or within the date range specified above. ALL refunds will be returned by check.

                              REGISTER EARLY FOR PROGRAMS
                              Nothing stops a program in its tracks faster than everyone waiting until the last minute to register. Many of our programs need a
                              minimum number of participants to run so a program may be canceled if there are not enough registrants. A minimum number
                              of participants must be reached at least one week prior to the program beginning.

					On occasion, we may take photos or video of individuals enrolled in programs or using parks facilities. Please be aware that
                              these images are for Township use and may be used in future promotional materials.

Briar Bush Nature Center is a rich oasis in the middle of suburban Abington Township. Our 12.5 acres feature over
a mile of trails winding through a mixed deciduous/coniferous forest. Whether inside the museum or out on the

                   Township) is a SENIOR group for those who live in

                   55 years of age. ARCAT began over 50 years ago and
                   is run by a staff of volunteers under the sponsorship
                   of the Abington Township Parks and Recreation.

participate in program(s) and/or events offered by the Township of Abington in Fall 2020 and Winter/Spring 2021.

I understand that participation in this Program involves risk of injury. I understand these risks are entirely my responsibility.
I expressly assume these risks. In particular, I am aware of the risks and hazards inherent upon entering a pool area and
I choose to voluntarily enter the premises, knowing the conditions might become more hazardous and/or dangerous for
myself and I voluntarily assume all such risks, loss, damages, or injury that may be sustained by entering the pool.

By signing this release of liability and participating in this Program, and in consideration for being permitted to participate
in the Program, I do hereby, for myself, my heirs, executors, administrators and assigns, and/or for any additional
family members and minors in my care for whom I am submitting a registration, fully and forever release, indemnify,
and discharge Abington Township and Abington Township Parks and Recreation Department, and their officers, officials,
departments, and other representatives and their heirs, administrators, successors and assigns, from any and all manner of
action, cause of action, suits, debts, accounts, controversies, damages, claims and demands whatsoever, which I or my legal
representative, and/or any minors in my care may have or acquire against Abington Township and Abington Township
Parks and Recreation Department, and their officers, agents and other representatives, by reason of any loss resulting from
personal injury or damage to personal property belonging to me and/or any minors in my care which may occur during or
by reason of my, and/or any additional family members’ or minors’ in my care, participation in the Program, whether the
same be known or unknown, anticipated or unanticipated. I fully and forever release and discharge Abington Township, the
Abington Township Parks and Recreation Department, and their volunteers and vendors, employees and agents from any
and all negligent acts and omissions in the same. I hereby grant Abington Township and any of its officials, officers, agents
and other representatives full authority to take whatever action they may consider to be warranted regarding my, and/or
any additional family members’ or minors’ in my care, health and safety, and I fully release Abington Township, its officials,
officers, agents and other representatives from any liability for such actions taken on my behalf or on behalf of any additional
family members or minors in my care.

I understand that no health and/or accident insurance is provided by Abington Township. I also understand and agree, for
myself, and/or for any minors in my care, that I am solely responsible for the cost and expense for furnishing medical or
other insurance to cover any expenses related to any such personal injuries or property damage.

I agree, for myself and/or for any additional family members or minors in my care, to comply with all Abington Township
rules and regulations, including any rules and regulations governing any activities or programs for which I, and/or any
additional family members or minors in my care, have registered, and understand and agree that noncompliance with any
such rules and regulations by me, or any minors in my care, may result in termination of the privilege to use Abington
Township facilities and/or participate in Abington Township-sponsored activities and programs. In the event of such a
termination for cause, I understand that I will not be entitled to be reimbursed for any registration, activity or program fees.

I grant Abington Township the right to use my/our name, image, photograph and video, including composite or
modification, representations in publications, brochures, newsletters, reports, website and any other material relating to
Township activities or programs. I waive the right to inspect or approve versions of my/our image used for publication or
the written copy that may be used in connection with the image.

COVID- 19 Assumption of Risk and RELEASE FORM

     The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely
     contagious and is believed to spread mainly from person-to-person contact. Abington Township has put in place preventative measures
     to reduce the spread of COVID-19 at the Parks and Recreation Department. However, Abington Township cannot guarantee that
     you or any minors in your care will not become infected with COVID-19. Further, participation and attendance with any Abington
     Township Parks and Recreation locations or activities could increase your, or any minors in your care, risk of contracting COVID-19.

     By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk for myself and/or for
     any minors in my care that may be exposed to, or infected by, COVID-19 by participation in Abington Township’s Parks and Recreation
     activity and that such exposure or infection may result in personal injury, illness, permanent disability, and death;
