Introduction

The oil and gas industry recognises that a major objective is to prevent incidents occurring but, if they do
happen, the priority is then to control and minimise their effect.

Emergency response and rescue vessels (ERRVs) play a significant role in responding to emergencies
offshore with their attendant fast rescue crafts and daughter crafts. OPITO has a suite of ERRV
standards of which the ERRV Crew Fast Rescue Craft Coxswain is one. This document sets out the initial
emergency response training requirements for ERRV Crew FRC coxswains which will be conducted at an
onshore training establishment (Section A).

Training establishments and employers should recognise that this is only part of a broader
training/assessment programme as OPITO approved FRC coxswains are continuously trained and
assessed based on the OPITO approved Ongoing Onboard Development and Training Programme for
ERRV Masters and Crews standard (OODTP).

To develop a training course, an evaluation of workplace competence is required. In other words, what
does an employee have to know and do if he/she were the designated FRCC on an ERRV? The following
were identified and developed for this role:

               What personnel are expected to do
               The underpinning knowledge and skills they would require to enable them to do what
                was expected
               How they could demonstrate what was expected of them
               How their performance would be assessed

The identified knowledge and skills are outlined in Section D. The employer and OPITO approved
providers of the OODTP can use this section to assess FRC boatmen in the training centre and the
workplace.

OPITO, as custodian, has a responsibility to the industry to maintain, develop and update this standard.

For the purpose of this document:

Rescue refers to the overall operations of recovering and transferring casualties

Recovery refers to getting the casualties out of the water (TEMPSC, liferaft etc.) and into the FRC

Transferring refers to moving the casualties from the FRC to the ERRV

      SECTION A         Fast Rescue Craft Coxswain Training

A.1     Fast Rescue Craft Coxswain Target Group

This programme is designed to meet the initial emergency response training requirements for an ERRV
Fast Rescue Craft Coxswain. Further vessel specific training is required.

A.2     Fast Rescue Craft Coxswain Delegate Pre-requisites

Attendance on this training course is open to delegates who have a valid OPITO Initial Training
Shipboard Operations certificate or reassessment to an equivalent standard through OODTP, a valid
OPITO ERRV Crew Fast Rescue Craft Boatman certificate and who have three months sea service as an
FRC boatman.

*Note: A current and valid Advanced Medical Aider may attend this course if they do not possess a valid
ITSO certificate. However in this instance they have to have a current ERRV OODTP for Masters and
Crew (OPITO Code 6190)

A.3     Fast Rescue Craft Coxswain Training Physical and Stressful Demands

All personnel who participate in practical activities specified in this standard must be capable of
participating fully.
OPITO-approved Centres are therefore required, as a minimum, to ensure that prior to participating in
practical exercises the delegate or candidate provides written confirmation that they deem themselves
capable of undertaking all aspects of the training or assessment. Centres must also make the
delegate/candidate aware that they must immediately inform the Centre staff if this capability changes
– at any time prior to, or during the programme.

Delegates/candidates must be advised that they are required to declare any current or pre-existing
medical conditions which may be exacerbated by, or impair their ability to complete, the
training/assessment programme. Should this be the case, the individual may be required to provide an
authentic medical certificate issued since any identified medical condition was diagnosed.

The OPITO-approved Centre shall keep a record of the delegate’s/candidate’s declaration of fitness in
accordance with their document control policy(s) or procedures.

This information, along with summary details of the type of physical activities the delegate/candidate
will be asked to perform, will be given to delegates/candidates by the OPITO-approved Centre and, if
applicable, to their sponsoring company as part of the joining instructions. The responsibility for
declaring any known current or pre-existing medical conditions that could have adverse effects to the
individual’s state of health while undertaking the training and/or assessment activities lies with the
delegate/candidate and/or company sponsoring the delegate.

Where doubt exists regarding the fitness of any delegate/candidate, the OPITO-approved Centre should
direct the individual to consult a medical officer familiar with the nature and extent of the training.

Note: Practical exercises should be designed and delivered solely to meet this standard, and must not
place on the delegates any demands other than those required to meet the Standard.

A.4    Fast Rescue Craft Coxswain Training Aim and Objectives

The aim and objectives of the FRC Coxswain training programme are to equip the delegate with the
initial knowledge, understanding and skills required to perform the role of ERRV Crew Fast Rescue Craft
Coxswain Note: Additional vessel specific training will be required.

A.5     Fast Rescue Craft Coxswain Training Learning Outcomes

Delegates following a series of explanations, demonstrations and opportunities to practise will be
assessed against the following learning outcomes:
To successfully complete this programme, delegates must be able to:

       (1)     Explain the roles and responsibilities of the FRC coxswain in ERRV activities
       (2)     Explain the effects and possible reduction of Whole Body Shock and Vibration and
               measures for reducing the risk to FRC crew
       (3)     Explain Basic Rules of the Road (IRPCS)
       (4)     Explain rescue processes from TEMPSC, liferafts, helicopters, personnel descending
               devices and rope access systems to the FRC
       (5)     Explain the hazards of accessing installations by FRC in an emergency
       (6)     Explain the requirement for installation specific risk assessments
       (7)     Explain the personal locator beacons and emergency location beacons that may be
               encountered when responding to an emergency and how they can be deactivated.
       (8)     Explain the principles behind the use of direction finding equipment
       (9)     Explain how direction finding equipment can be used to locate casualties
       (10)    Demonstrate leadership and team working capabilities
       (11)    Demonstrate facilitation capabilities
       (12)    Demonstrate pre-launch and re-launch checks on the FRC
       (13)    Demonstrate how emergency equipment should be used when needed
       (14)    Demonstrate establishing and maintain communications
       (15)    Demonstrate directing the FRC crew in all activities
       (16)    Demonstrate confirming the FRC is ready for launch and start the engines
       (17)    Demonstrate FRC handling techniques required for safe launching and release
       (18)    Demonstrate handling both jet and outboard FRC’s in confined spaces
       (19)    Demonstrate handling both jet and outboard FRC’s in open waters
       (20)    Demonstrate acting appropriately during and after a capsize and during the righting

               jet and outboard FRC’s
       (23)    Demonstrate directing the crew for casualty recovery
       (24)    Demonstrate towing another vessel of equivalent size to the FRC

Learning Outcomes 10 through 26 are to be assessed during practical exercises.

A.6     FRC Coxswain Training Delegate Performance Assessment

Delegates will be assessed against the learning outcomes specified in section A.5 using direct
observation and oral and/or written questions as appropriate.
Assessment of delegates will be by continuous observation and evaluation. The purpose of continuous
assessment is to ensure delegates are acquiring the necessary skills, knowledge and understanding of
the FRC coxswain subject matter being covered. This will be achieved by direct observation of practical
work and by questioning based on the learning outcomes in A.5.

If required, further time and opportunity for coaching will be given to delegates to enable them to reach
the standard leading to the award of a certificate. However, the time available for this during the
programme is limited and it should be recognised there may be occasions where other arrangements
need to be made.

A.7     Fast Rescue Craft Coxswain Training Duration, Timing and Handout

The optimum 'contact time' for this training is seen as 32 hours. It is suggested that a ratio of 40%
theory to 60% practical is appropriate.

The total contact time per day shall not exceed 8 hours. The total training day shall not exceed 10 hours
(the total training day includes contact time, coffee and meal breaks and travel between training sites
where applicable).

*Contact time includes the delivery of the theoretical and practical training/assessment programme.

**The total programme day includes the delegate enrolment and certification process, contact time,
welfare breaks, meal breaks and where applicable, travel between sites.

Delegates should be issued with course notes for Fast Rescue Craft Coxswain training when the course
commences. These notes should be suitable for use as a reference manual during and following the
course. The manual should be supported with supplementary information covering the following topics:

              Leadership
              Teamwork
              Facilitating
              Appraisal techniques

    A.8          Fast Rescue Craft Coxswain Training Programme

The FRC Coxswain Training Programme specified below is designed to help delegates achieve the stated
learning outcomes specified in Section A.5.

To make efficient use of time and ensure effective learning, there should, wherever practicable, be an
integration of the three phases of explanation, demonstration and practise. Full use should be made of
audio aids, visual aids and course handout material.
Prior to the start of each module, the following must be included as part of the introduction by training
staff:

        (a)     Aim                      The main purpose of the module
        (b)     Learning Outcomes        What the delegates are expected to learn
        (c)     Timetable                Training module duration and timing
        (d)     Assessment                How delegates will be assessed and what they will be
                                          assessed against
        (e)     Staff                     Who will be delivering the training and roles of training
                                          support staff.

The FRC Coxswain Training Programme comprises the following module and elements:

Module 1        Rescuing and Recovering Casualties using a Fast Rescue Craft
Element 1.1     Introduction to Fast Rescue Craft Coxswain Responsibilities
Element 1.2     Preparing the FRC
Element 1.3     Launching, releasing and handling the FRC
Element 1.4     Recovering casualties
Element 1.5     Transferring casualties and retrieving the FRC to the ERRV
Element 1.6     Leadership, team working and facilitation
Element 1.7     Actions in a capsize

In order to make efficient use of the time and ensure effective learning there should whenever
practicable be an integration of the three phases of explanation, demonstration and practice. The
training programme outlined below will assist delegates to meet the stated outcomes.

Note: Each delegate must be given the opportunity during the practical elements of this programme
to use radios and correct radio procedures.

Each delegate must be given the opportunity to practice FRC handling in both outboard and jet FRCs

MODULE 1             RESCUING CASUALTIES USING A FAST RESCUE CRAFT

ELEMENT 1.1          Introduction to Fast Rescue Craft Coxswain Responsibilities

Training Staff to explain:

1.1.1        The role and responsibilities of an FRC coxswain within ERRV operations
1.1.2        Hazards of Whole Body Shock and Vibration and the coxswain’s role in minimising the risk
             of injury including the importance of correct seating positions
1.1.3        Craft maintenance and readiness
1.1.4        Basic Rules of the Road (International Rules for Prevention of Collisions at Sea IRPCS) to
             include:

              (a)    Power driven vessels meeting head on, in a crossing situation and overtaking
              (b)    Lights for Power Driven Vessel which is under 50 metres
              (c)    Lights for Power driven Vessel which is probably over 50 metres

ELEMENT 1.2          Preparing the FRC

Training Staff to explain and demonstrate, delegates to practice:

1.2.1        Pre-launch and re-launch checks on the FRC
1.2.2        Use of emergency equipment
1.2.3        Establishing communications
1.2.4        Directing the crew
1.2.5        Confirming that the FRC is ready to launch

ELEMENT 1.3          Launching, releasing and handling the FRC

Training Staff to explain and demonstrate, delegates to practice:

1.3.1        Starting the engines
1.3.2        Directing the crew in all activities
1.3.3        Maintaining communications and the correct use of VHF radio
1.3.4        FRC handling techniques required for safe launching and release
1.3.5        FRC handling techniques when in displacement mode in open sea conditions
1.3.6        FRC handling techniques when in planing mode in open sea conditions
1.3.7        FRC handling techniques when in displacement mode in confined areas

Note: Delegates must be given the opportunity to practice Element 1.3.5, 1.36 and 1.37 in both jet and
outboard FRCs.

ELEMENT 1.4          Recovering casualties

Training Staff to explain:

1.4.1        Recovering casualties to the FRC from situations such as TEMPSC, liferafts, helicopters,
             personal descending devices, escape systems and rope access systems
1.4.2        Accessing installations and the typical hazards that are present when working close to or
             underneath an installation.
1.4.3        The need for an installation specific risk assessment including those involved and the
             considerations to be made.
1.4.4        The types of personal locator beacons and emergency locator beacons that may be
             encountered in an emergency and how they are operated
1.4.5        The principles behind direction finding equipment found onboard the ERRV, FRC and DC
             and it’s use in locating casualties

Training Staff to explain and demonstrate, delegates to practice:

1.4.6        Assessing the prevailing conditions (including the effects of tide/current in operations)
1.4.7        Directing the crew
1.4.8        Maintaining communications
1.4.9        Locating a casualty using basic SAR techniques to include search patterns
1.4.10       Approaching a casualty
1.4.11       Casualty assessment prior to rescue
1.4.12       Directing the crew for casualty recovery
1.4.13       Practical towing of another vessel (demonstrate using another a craft of similar size) to
             include information on different towing arrangements and safe towing techniques

Note: Delegates must be given the opportunity to practice Elements 1.4.10, 1.4.11 and 1.4.12 in both
jet and outboard FRCs.

ELEMENT 1.5          Transferring casualties to the ERRV

Training Staff to explain and demonstrate, delegates to practice:

1.5.1        Maintaining communications
1.5.2        Transfer of casualties to the ERRV (the ERRV may be simulated)
1.5.3        Retrieval of the FRC to the ERRV(the ERRV may be simulated)

Delegates to practice:

1.6.3        Facilitating a short training session and providing appraisal at its conclusion
1.6.4        Working as team leader and ensuring the boatmen work as a team

ELEMENT 1.7           Action in a Capsize

Training Staff to explain:

1.7.1        The common causes of FRC/DC capsize and the hazards encountered in the event of a
             capsize
1.7.2        Details of capsize specific equipment on the FRC including the self-righting system and the
             method of activation, the rescue line and quoit and typical engine cut out systems.
1.7.3        Actions to take as a coxswain in the event of capsize including initial action, exiting the FRC,
             use of rescue line and quoit, activating the righting system and re-boarding the FRC

Training Staff to explain and demonstrate, delegates to practice:

1.7.4        Bracing for capsize
1.7.5        Initial action after capsize including actions to take as coxswain
1.7.6        Exiting the FRC and the importance of staying in contact with the FRC
1.7.7        Using the rescue line and quoit
1.7.8        Activating the self-righting system (The self-righting system may be simulated but must be
             used in at least one of the exercises)
1.7.9        Re-boarding the FRC and the necessary checks.

Note: Element 1.7 must be carried out in a controlled pool environment with water temperature at 15
degrees Celsius or above.

      SECTION B            Resources

In order that a training programme may be delivered successfully it is essential that the right people are
there to support the programme and that the appropriate facilities and equipment are in place.

B.1                Staff

Instructional staff will be required to carry out training and to evaluate the delegates’ performance.

It is important that the staff involved have the appropriate qualifications and experience to ensure that
all training and supporting activities are carried out in accordance with current legislation.

All instructional staff must be:

      (a) Able to demonstrate, via their qualifications and experience, competence in FRC Coxswain
          operations including command and control, Rules of the Road, signals, lights and fast rescue
          boat handling

      (b) Trained in instructional/lecture techniques and/or have proven instructing/teaching experience

      (c) Hold an industry recognised assessor’s qualification.

      (d) Included in an ongoing staff training programme, which includes visits to operational ERRVs, to
          enable them to maintain and update skills and knowledge

B.2           Trainer/Delegate Ratio

It is recommended that the maximum number of delegates attending this programme is twelve.

The following ratios indicate the maximum number of delegates to be supervised by one instructor at
any one time during each activity.

                  Theory                          1 : 12
                  Practical & Demonstration       1 : 4 per craft
                  FRC Capsize                     1: 4 (one instructor to be nominated pool supervisor
                                                  and a minimum of two staff as in-water safety persons)

B.3              Facilities

Different facilities may be required for this training programme and it is important to make sure that the
following are available:

Administration arrangements to support delegates from booking through registration and assessment
to certification.

Theory training area(s) so designed to enable each delegate to view, hear and participate fully in the
subject matter being taught.

Practical training areas must simulate the layout, equipment and conditions found on an ERRV for
operating an FRC. Also required is an suitable and sufficient area where fast rescue craft manoeuvring
exercises can take place. Delegates must also be able, individually or as part of a team, to view, hear
and practise the activities identified in the training programme.

FRC capsize training area to be a controlled pool environment with water temperature at 15 degrees
Celsius or above.

All facilities must be maintained, and where appropriate, inspected and tested in accordance with
current standards/legislation.

B.4           Equipment

The following equipment, which must be of a type substantially representative as detailed in the latest
ERRVA Guidelines, must be available for use and maintained to ensure effective operation/use during
training sessions.

        (a)     PPE
        (b)     FRC and ancillary equipment – outboard and jet. Both must have adequate seating for
                all delegates and instructor
        (c)     FRC launch and recovery system
        (d)     casualty recovery devices
        (e)     casualty handling devices
        (f)     safety rescue craft
        (g)     radios
        (h)     simulated casualties

All equipment must be maintained, and where appropriate, inspected and tested in accordance with
current standards/legislation.

      SECTION C          Administration and Certification

C.1               Joining Instructions

All joining instructions must contain information which indicates that certain aspects of the course are
of a physical nature and contain potentially stressful elements.

Prior to each course commencing, delegates must sign a declaration indicating they have read and
understood a written statement regarding the physical and potentially stressful nature of the
programme, and the need for delegates to be in good health.

C.2               Periodicity

Training establishments and employers should recognise that this is only part of a broader
training/assessment programme as OPITO approved FRC coxswains are continuously trained and
assessed based on the OPITO approved Ongoing Onboard Development and Training Programme for
ERRV Masters and Crews standard.

C.3           Certification

Training Centres are responsible for issuing a certificate direct to the delegate completing the
programme and to the sponsoring company (when required). Each certificate must indicate that the
delegate has been assessed against and met the learning outcomes and must contain the following:

        (a)      Training Centre name
        (b)      Full OPITO course title stating that it is OPITO-approved
        (c)      OPITO registration code

Please note: If the expiry date on the delegates previous certificate is within 3 months of the course
enrolment date then the date of the new certificate should correspond with the expiry date of the
existing/previous certificate unless stated otherwise by the Duty Holder or Asset Owner or Operator.

C.4              Course Administration

Each delegate attending any OPITO-approved programme must be registered with the Central Register
(CR) operated by OPITO. Registration must be made by the training centre to OPITO within one week
following the course.

OPITO confirms that information on the registration form will be contained in a computerised register
which will be available to employers, prospective employers and training providers in the oil and gas
industry to verify training records. At all times, use of this data will be strictly in accordance with
principles laid down in data protection legislation.
