Who are we?
   We work at the Fund Office
   Fund Office staff responsible for your fringe benefits
   The Fund Office is located on the District 1 campus
   Responsible for collecting employer contributions and allocating to employee’s
    individual account
   As a self-insured entity, the Fund Office currently processes all medical claims
   Dental claims are adjudicated by Delta Dental of Illinois (Not applicable to the
    Bronze PPO plan)
   Pension department processes all pension applications
   We partner with Fidelity to administer the Retirement Enhancement Fund
   Fund Office issues Vacation Savings Fund checks

What are my Fringe Benefits?
Your employer is responsible for paying a specified dollar amount per hour for each
fringe benefit included under your collective bargaining agreement (CBA).

Take the time to read your CBA to become familiar with the terms of your fringe
benefits.

Becoming Indentured
• You will become indentured within 30-45 days from the date that you
  are dispatched and start working
• At the time you are indentured, you will need to pay your annual
  initiation dues
• Keep your dues receipt with you until you receive your apprenticeship
  card

• Once you are indentured, schedule your DOT Physical!
   • FREE at the Operators’ Health Center or Activate Healthcare
      • Refer to information sheet in folder for phone numbers
   • You MUST bring your initiation dues receipt
   • You MUST call either location to make the appointment and identify yourself
     as being an apprentice at the training site
   • If you cannot make your appointment, please call to cancel and then re-
     schedule  Don’t be a no show

When Am I Eligible for H&W Coverage?

• During Administration Period, Fund Office will validate your
  dependents and select the appropriate coverage tier. Be sure to
  submit required documents.
• If you never reach 300 hours, you will not become eligible and you
  will not be able to use your credits for coverage.

What will I receive from the Fund Office?
• Once you meet eligibility requirements (i.e. 300 hours in 12-month period)
   • You will receive new member packet from Member Services
        You will receive a temporary vendor/medical ID card to use until you
         receive your official vendor/BCBS medical ID card
        Official BCBS medical ID cards will be issued within 30 days; if not received,
         call Member Services (708) 579-6600
   • Member Services will validate your dependents and update your Coverage Tier
     (Member, Member +1, Family)
   • You will receive welcome packets from vendors:
        ERS, Member Assistance Program
        OptumRx
        BCBS Card – medical ID card
        RedCard – vendors applicable to Bronze PPO Plan with informational inserts

Who are Eligible Dependents?
 • Your legal spouse

 • Physically or mentally disabled children

If you do not submit your required documents for your dependents within 90 days from your
eligibility date, you will not be able to add your dependents until the following Open Enrollment
Period mid-Jan. 2022 – 2/28/2022. Start gathering documents now!!

Why am I Auto-Enrolled into the Bronze PPO Plan?
• Lowest credit cost plan
• Easier to remain eligible
    No credits No eligibility  No medical or Rx Coverage
    Re-establish by working 300 hours in 12-month period
• Allows you to build up Credit Bank
• Become an informed consumer & have a better understanding of the Marketplace
• Take advantage of FREE services
• Use your Family Supplemental Benefit for services not covered by the plan

How Long am I in the Bronze PPO Plan?
• Upon your initial eligibility or if you re-establish eligibility, you will
  remain in the Bronze PPO Plan for the remainder of the Plan Year
• If you remain eligible at the time of Open Enrollment, you will be able
  to select from one of the seven health plan options (including the
  Bronze PPO Plan)
• If you first become eligible or re-establish eligibility on March 1, you
  will remain in the Bronze PPO Plan for 13 months, but you can select
  a Plan during the following year’s open enrollment period

• Member becomes eligible on May 1, 2021
   Will remain in the Bronze PPO Plan through March 31, 2022
   During open enrollment, mid Jan. 2022 – February 28, 2022, member can
    select new health plan option for coverage effective April 1, 2022 through
    March 31, 2023
• Member becomes eligible on February 1, 2021
   Will remain in the Bronze PPO Plan through March 31, 2021
   During open enrollment, mid Jan. 2021 – February 28, 2021, member can
    select new health plan option for coverage effective April 1, 2021 through

      o Ensure you have enough credits and project work hours to maintain
        higher credit costing plan
      o If you lose eligibility, you will auto enroll into the Bronze PPO Plan
• Member becomes eligible on March 1, 2021
   Will remain in the Bronze PPO Plan through March 31, 2022
   During open enrollment, mid-Jan 2022 – February 28, 2022, member can
    select new health plan option for coverage effective April 1, 2022 through

What if my spouse also has medical coverage?
• As long as you have Credits in your Credit Bank and you meet the
  Eligibility requirements, you must have coverage under the
  Marketplace
• Your coverage will be processed as primary, and your spouse’s
  coverage will be processed as secondary
• If your child is covered under both plans, the parent whose birthday is
  earlier in the Calendar Year shall pay as primary
    Different rules apply if you are divorced or not married

What if I have VA benefits?
• Again, as long as you have Credits in your Credit Bank and you meet
  the Eligibility requirements, you must have coverage under the
  Marketplace
• Your coverage will be processed as primary and then any remaining
  portions will be considered through your VA benefits

Why do I need to attend an Open Enrollment Event?
• Open Enrollment is an annual process – READ your open enrollment guide
  Open Enrollment: January 17, 2022 through February 28, 2022
  Plan Year is April 1 – March 31
• This allows you to review all the health plan options with a Fund Office navigator
• Your opportunity to select a new health plan option/coverage tier for the
  upcoming Plan Year (4/1/2022 – 3/31/2023)
  If you do not actively select a new health plan option, you will default back into
   the Bronze PPO Plan
• See the impact on your credit bank and ensure that you can maintain your health
  & welfare coverage

Fund Office will schedule a date to come to ASIP to assist members
with the enrollment process (typically, first Friday of February 2022).
Alternatively, you can schedule an appointment at one of the following
