Elder Resource Association
South of the River
Elder Resource Association (ERA) is a group of
professionals in the South of the River market
dedicated to improving the lives of seniors. We
meet regularly to network and find ways to better
serve our community. This guide features senior
related services in cities south of the river like:
Apple Valley, Burnsville, Eagan, Farmington,
Lakeville, Prior Lake, Savage, and others.
This is a collaborative effort to provide a
comprehensive collection of the services
available to seniors and their families. Please note,

22Home Care
Home care agencies provide care wherever a person
calls home: the home they own, an apartment, a
relative’s home, independent living, group home or
assisted living. Home Care Services can be arranged
for as little as a few hours a week to as much as 24-hour
care. Live-in caregivers are also an option.
Some in-home care services may be covered by
Medicare, veteran’s benefits, long-term care
insurance or other insurances such as supplemental
health insurance, worker’s compensation or even auto
liability insurance. These services can consist of
physical therapy, speech pathology, skilled nursing,
occupational therapy, home health aide, and/or
social work.
The purpose of Medicare-covered services is to teach
and train the patient and/or their caregiver(s) on
activities that will allow them to live independently in
their home. The activities focused on usually relate to:
living with a new diagnosis, assessing for home safety,
teaching exercises to improve strength due to illness or
surgery, teaching about new medications or diet, and
assisting with tasks such as bathing, wound care or
changing of bed linens. Once the established goals
are reached or you plateau and it is determined that
teaching, training, and rehabilitation are complete,
23the services, covered by insurance, will end. These
services are often combined with privately paid home
care as a supplement.
Non Medicare covered services are personalized to
the individual needs, ranging from simple assistance
around the home all the way through end-of-life care
at home. Services can include help with activities such
as: preparing healthy meals, housekeeping, bathing
and personal care, medication, transportation,
running errands, and social outings. Services also can
include general supervision for memory care and
companionship.
A home care provider is any entity that regularly
engages in the delivery of home care for a fee. At
least one home care service must be provided directly,
although additional home care services may be
provided by contractual arrangements.
A home care provider must have a current license to
operate in the state. Minnesota has two types of
home care licenses. The license a home care provider
holds depends on the types of services offered. In
Minnesota, home care licenses are either basic or
comprehensive.
Basic Home Care Licensure per the Minnesota
Department of Health is defined as: providing
assistance with dressing, self-feeding, oral hygiene, hair
care, grooming, toileting, and bathing, providing
24standby assistance within arm’s reach for safety while
performing daily activities, providing verbal or visual
reminders to take regularly scheduled medication,
providing verbal or visual reminders to the client to
perform regularly scheduled treatments and exercises,
preparing modified diets ordered by a licensed health
professional and laundry, housekeeping and other
household chores including meal preparation and
shopping.
Comprehensive Licensing includes all of the Basic
licensed services plus: Advanced Practice Nurse
services, Registered Nurse services, Licensed Practical
Nurse services, Physical Therapy, Occupational
Therapy, Speech Language Pathologist services,
Respiratory Therapy, Social Work services, Services by a
dietitian or nutritionist, complex and specialty
healthcare, medication management, delegated
tasks to unlicensed personnel and hands-on assistance
with transfers and mobility and providing eating
assistance for clients with complicating eating
problems.
When gathering information on home care providers,
it is important to determine if they are licensed,
bonded and insured. Generally, an assessment will be
done to determine services and eligibility of coverage.
Check with providers to determine the details specific
to your situation, the services that they can provide,
the licensure they carry and the coverage available.

There is a multitude of senior living options. Choosing
the right one for you or a loved one depends on needs
and personal preferences. Here is a short summary of
options generally available in all areas. Please note
that in some cases, facilities may go by other names
depending on state specific rules as well as provider
preferences so by all means, ask when you visit.
INDEPENDENT LIVING
An independent living community is typically a multi-
unit senior housing development that provides full
apartment style living. Independent living communities
may also offer supportive services such as meals,
housekeeping, social activities, and transportation.
Some names these communities go by include senior
housing, cooperatives, condominiums, and subsidized
housing.
ASSISTED LIVING
Assisted living facilities are for people needing
assistance with Activities of Daily Living (ADLs) but
wishing to live as independently as possible for as long
as possible. Assisted living can be found in community
or residential settings. Assisted living exists to bridge the
gap between independent living and nursing homes.
Residents in assisted living are not able to live by
themselves but do not require constant care either.
41Assisted living facilities offer help with activities such as
eating, bathing, dressing, laundry, housekeeping, and
assistance with medications. Assisted living is not
necessarily an alternative to a nursing home, however
some assisted living communities offer care suites. Care
suites are generally a designated area specializing in
higher levels of care. Each assisted living community
offers an assessment prior to move in to determine
appropriate level of care.
MEMORY CARE AND ALZHEIMER’S CARE
Certain assisted living facilities offer specialized
environments for those with cognitive impairment. The
environments offered are designed for safety, comfort
and familiarity. All staff are specially trained to work
with people who have memory loss.
LONG TERM & TRANSITIONAL CARE
A nursing home is a long-term care facility licensed by
the state. Nursing homes offer room and board and
health care services, including basic and skilled nursing
care, rehabilitation, and a full range of other therapies,
treatments, and programs. Residents of nursing centers
require regular health monitoring and ongoing rather
than intermittent nursing care. Some nursing facilities
have designated units that serve individuals who
require more acute or specialized services such as
rehabilitation.
42RESPITE CARE
Respite care can provide short-term or temporary
assistance ranging from a few hours per day up to
several weeks depending on the need. The goal is to
provide relief, or respite, to the regular caregiver,
usually a family member. Most assisted living and skilled
nursing communities will provide respite options if
accommodations are available. Home care agencies
may also be able to provide similar services in the
home.
