OUR REFINED
Hand Heart Pocket recently embarked on a
commitment to help improve outcomes for youth
at risk for at least the next three years. This saw
us start to direct the majority of our Significant
PHILANTHROPIC and Flagship Grant funding towards programs and
initiatives that aim to help at-risk youth to overcome
the significant challenges they may face. Our Theory
of Change explains the problem we are trying to

Young people at risk through trauma, disadvantage, social isolation and discrimination, are more likely to have adverse outcomes over
