                                           Home Based Business
                                           Step 1: Download HHB application from MWR website.

                                           Step 2: Complete the Application.
                                           This includes visiting and obtaining approval from
                                           installation agencies like the housing office and community
                                           housing manager. The application should identify the
                                           agencies, but ask your MWR representative if you have
                                           questions. If you need any additional documentation (like
                                           licensing or certifications), get that, too.

                                           *Please note that Liberty & Chatham Counties do not issue
                                           business licenses to on-post residents.

                                           Step 3: Turn in the Application:
                                           Once you’ve completed your paperwork, submit the
                                           application to your Garrison MWR. Keep a copy of the
                                           completed application for your own records.

Requirements to renew a Student ID Card
 Full-time College students between the ages of 21 and 23
  Enrolled full-time in a college for a degree (can’t be an unaccredited college or a
   technical certificate)
  Sponsor must be providing 50% or more support
 To prove that your student is enrolled as a full-time student, they’ll
  need to get a letter from the College Registrar. The letter must contain
  the following information:
    1. They are enrolled as a full-time student
    2. Estimated graduation date
    3. The student is in an associate’s degree or higher program
    4. The current semester dates (ie: Spring 2022-1/15/22-5/9/2022)

                                         Annual Exams Reminder

            - Annual exams: review labs, medications, and preventative medicine efforts
            - Mammograms: recommended annually for over 40 and appts can be made
            without referral
            - Colon cancer screening: recommended starting at age 45. Colonoscopy is
            the gold standard; however, the PCM can order a FIT test which is a lab that
            can be completed annually
            - Pap exams: every 3-5 years for women over age 21-65. Make an appt with
            your primary care provider
            - Osteoporosis screening: recommended for all women >65 years old
            - For Diabetics: annual A1C testing is recommended

            - Immunizations other than COVID (walk-in):
            Tetanus: recommended every 10 years
            Zoster: recommended for all patients at the age of 50

            *review all medications at home to ensure expiration date and refill status
