                                Good news!
As part of the efforts to make Sprint a Great Place to Work, there are no increases to
 2020 medical plan, dental, vision and life insurance, dependent life insurance and
                                  AD&D premiums.

                Review the information in this document carefully.
Use all the resources available to make educated benefits decisions for yourself and
                                    for your family.

 You can choose from three national medical plans:

       Core plan with Health Savings Account
       Basic plan with Health Savings Account
       Health Account plan with Health Reimbursement Account
       Kaiser HMO, in select regions

       Changes to Health Account Plan Emergency Room Co-Payment:
        Emergency room co-payment will be increased to $150 under the Health
        Account Plan.
       No other changes to apply to any of these Plans.

       New health management programs:
        o Hypertension: Program offers a simple, advanced blood pressure
          monitor combined with the power of personalized coaching.
              UHC partners click on CVS/Livongo for more information
              BCBSIL partners click on BCBSIL/Livongo for more information
          Registration information will be available after January 1, 2020.

        o   Hinge Health digital program to help manage chronic
            knee/hip/back/chronic conditions for BCBSIL partners.

       Changes to Kaiser California, Georgia and Mid-Atlantic states:
        Prescription Drug Coverage: $250 deductible now applies to all covered
        drugs for a 30-day supply; Specialty Drugs now have a 30% coinsurance
        (after deductible), not to exceed $200 for a 30-day supply

       Changes to Kaiser Hawaii Inpatient Services: 10% coinsurance added to
        inpatient hospital services

 No changes, except for ongoing formulary changes, as determined by CVS
 Caremark.

 Sprint’s Health Savings Account (HSA) will continue to be administered by
 HealthEquity.

 New HSA maximum contribution amounts (including Sprint company funding):
    Individual: $3,550 ($50 increase)*
    Family: $7,100 ($100 increase)*

 *Employee partners age 55 or older are eligible to make an additional “catch-up”
 contribution of $1,000 each year.

 Your current participation and annual contribution election (if applicable) will
 automatically continue at your contribution for 2020 unless updated through your
 active enrollment submission.

 Reminder – Sprint contributes to your HSA:
 If you contribute just $24 per year to your HSA, Sprint contributes:
      Up to $500 toward your individual maximum contribution,
      Up to $1,000 toward your family maximum contribution.

 Health Care FSA: New maximum annual contribution amount $2,700 – a $50
 increase.

    Onsite Health Center (Reston and Overland Park): changes to fees for various
     services, click here for more details.

    Health Plan Cost and Care and Provider Support tools:
       o Castlight Health will be discontinued as of January 1, 2020.
         o   Cost, Care and Provider tools will be provided by your medical carrier
             (BCBSIL or UHC) and CVS Caremark websites.
         o   Register on vendor websites/apps to take full advantage of all of the
             features available such as:
                   locating network providers
                   ID cards
                   claims and
                   comparing costs and quality for the services available

    Vision Plan: Davis Vision is implementing a new nation-wide lab network to
     manufacture and dispense eyewear from a vastly expanded catalogue. The lab
     network offers access to high quality eyewear using advanced technology and
     broad choice in lens functionality. Members can choose from a larger variety of
     lenses, including the most utilized and requested digital progressives, anti-
     reflective coatings, and blue light protection.

                                                                      The HLD status
 you have in 2019 carries forward unless your HLD status has changed and you
 submitted the required activities by the Oct. 4 deadline to take advantage of greater
 medical premium discounts. IMPORTANT: If you actively enroll and your HLD status
 changed, you will be required to take ACTION to select a medical plan and enroll
 your dependents (as applicable).

 Confirm your medical surcharge designation is correct: Your current election will
 carry forward, and you don’t need to re-designate your status unless there is a
 change.

                 If you are enrolled in a Flexible Spending Account (Health Care
 and/or Dependent Care), you’ll need to actively enroll during Annual Benefits
 Enrollment and select the amount you wish to contribute in 2020.
    o

                                        do not need to be re-elected. However, you’ll
 need to enroll if you wish to change your contribution amount OR if you actively
 enroll and you have either a carrier medical and/or healthy living discount change.

 Your election will carry forward.

                                    Once you’ve enrolled, you’ll receive a confirmation
 statement via email after one business day. Be sure to review your statement
 carefully. Enrollment on the last day of Annual Enrollment will not allow you to

receive your confirmation statement in time to make any changes if you find errors
 in your intended elections.
