Mandatory use of registration name badges will be enforced for admittance to all conference events,
meals, sessions, and Exhibit Hall.
Method of payment should be specified on the registration form.
Registrations must be postmarked by March 13, 2019 to avoid a $50 administration fee.
Cancellations:
All cancellation requests must be in writing. Requests can be e-mailed (prwa@prwa.com), faxed or mailed.
A $50 administrative fee will be charged for requests postmarked after March 13, 2019.
NO refunds will be given after March 20, 2019.
Consent for Use of Photographic Images:
Your registration and participation in any Pennsylvania Rural Water Association (PRWA) function
hereby grants PRWA to take and use: photographs and/or digital images for use in news releases and/or
educational materials. These materials might include printed or electronic publications, websites, and
other electronic communications. Your name and identity may be revealed in descriptive text or
commentary in connection with the image(s). No compensation will be given for use of images. All
negatives, prints, and/or digital reproductions will be property of PRWA.

Every System Member with a population of 15,000 or less is considered
a voting member. Please take a few minutes to complete this form to
let us know who will be the Designated Delegate to represent your
system’s vote at the Annual Business Meeting which will begin at
10:00 am on Tuesday, March 26, 2019 at the Annual Conference.
